Physical Information

All Athletes must have completed the following paperwork to be scheduled into the Athletic Period:

1. Physical Examination from your doctor and medical history
2. Athletic Emergency Travel Card
3. Sportsmanship Standards
4. UIL Acknowledgement of rules
5. Athletic Participation Consent form
6. Copy of Birth Certificate

All paperwork is due by May 23, 2008.  All copies of the necessary paperwork can be found on CFISD website under departments/ athletics.

All girls may purchase their Athletic Packet during Eagle Express ( Sat. August 16- 7^th grade 12-2 p.m. & 8^th grade 2-4 p.m.) The packet includes an athletic uniform, athletic backpack and towel fee.

 7^th  and 8^th  grade girls Volleyball try-outs will be held August 26, 27 & 28 at 6:30 a.m. All girls must have a physical on file to try- out.

7^th and 8^th grade boys and girls Cross Country season will begin August 29 at 6:30 a.m. All athletes must have a physical on file to participate.
